Unfortunately you cant amend a submitted online admission form. Please contact your hospital directly and they will update your admission paperwork for you.

You need to enter the information as per what is on your medicare card.

Please check your birth date is correct
Medicare number: Please ensure you have entered the numbers correctly.
First name: Only enter the first name on your medicare card, no middle name or middle initial
Surname: As per medicare card.
Expiry date: Ensure this is also as per your medicare card
If you still have problems the form will let you progress without verifying your medicare card, Our hospital team will verify the card once it is submitted.

Go to the My Account tab at the top of the screen and then click on Edit to update your contact details and save.
Click on Change Email to update your email details then click on confirm. This will generate an email to the new email address with a link to confirm.
Go to your email inbox and click on this link and enter password.
