Personal Information Collection Statement

Ramsay Health Care (‘Ramsay/we/us’) is bound by the Australian Privacy Principles under the Privacy Act 1988 (Cth) and other relevant laws about how private health service providers handle personal information. Ramsay is committed to complying with all applicable privacy laws which govern how Ramsay will collect, store, use and disclose your personal information.

This Collection Statement and the Ramsay Privacy Policy set out the way in which Ramsay will handle your personal information. The Ramsay Privacy Policy also sets out the way in which you may seek access to or correct information that we hold about you. You can view the Ramsay Privacy Policy here.

Your personal information is being collected for the primary purpose of understanding the experience of our patients to improve the services we offer (‘Primary Purpose'). By participating in the survey, you acknowledge and agree to the terms of this Collection Statement. If you choose to participate in the survey, we may link personal information we hold about you with your survey response such as your first name, date of birth, contact details, gender and/or birth sex, type of service or appointment, discharge date, and appointment date. We may also collect device data such as your computer or mobile device’s operating system type and version, browser type, IP address and general location information such as city, state or geographic area.

Your personal information will not be used or disclosed for any purposes other than the Primary Purpose unless you have consented to this (for example, if you state in your survey response that you wish to be contacted about your experience).

Your personal information collected for the Primary Purpose will be retained by Ramsay and stored securely in accordance with the Ramsay Privacy Policy and as required by legislation. Your personal information will be permanently deidentified or destroyed once it is no longer required for the purpose for which it was collected.

We use a third-party service provider, Qualtrics, to securely collect and store your information as part of the Patient Experience Survey. The Qualtrics Privacy Policy can be found here: https://www.qualtrics.com/privacy-statement/.
