A/Prof Underhill completed his Bachelor of Medicine and Surgery in 1987 at Melbourne University.  He became a Fellow of the Royal Australasian College of Physicians in 1997. 

In the mid 90's A/Prof Underhill worked as the Senior Clinical Research Registrar at Guy's Hospital in London.

A/Prof Underhill was a founding partner of Border Medical Oncology in 1999.
